The Registered Nurse RN with Homecare is a professional caregiver who assumes responsibility and accountability for assessing planning implementing and evaluating care of HospiceHome Health adult and pediatric patients and their families in their home setting.
The RN provides the best clinical practice in an environment of caring and healing while working autonomously in the home.
Utilizing the nursing process the RN delivers care in compliance with hospicehome health policies and procedures legal and regulatory standards.
The RN utilizes the nursing process by use of technology therapeutic intervention evidencebased practice and coordination of care with other health team members.

Performs appropriate assessment of physical social and psychological status including cognitive communicative and developmental skills as appropriate.

Uses assessment information and critical thinking skills to collaboratively develop an individualized plan of care.
Revise and reassess changes as appropriate.

Actively seeks patient family team and physician involvement to develop plan of care including needed education.

Collaboratively plans and prepares patients for further support needs once the current episode of care outcomes are met i.
e.
teaching referrals and followup.
.
Sets priorities coordinates and organizes patient care delivery through effectively managing time supplies and resources.
Minimum Qualifications Note RNs hired on or after January who have months or less of working experience as an RN prior to joining Intermountain will be required to obtain their BSN degree on or before June.
Preferred Qualifications.
Estimated Salary: $20 to $28 per hour based on qualifications.
